Central London Community Healthcare NHS TrustBrent Urgent Community Response Service is a national leader in intermediate care that aims to bring the best of hospital care to patients\'' homes, therefore avoiding unnecessary hospital attendances and admissions. Most of our referrals come from GPs, London Ambulance Service, and Accident and Emergency departments. Attending in pairs, we perform an initial assessment at home, conducting a holistic evaluation of the patient and in some cases starting a treatment plan within 2 hours of referral. The team can utilise a number of pieces of diagnostic equipment in the home, plus we have access to hospital facilities for various scans when required. We also provide our services in AandE to avoid hospital admissions and facilitate safe discharges. Brent Urgent Community Response looks to provide the very best care, aiming to deliver the same standard a patient would receive as an inpatient. This is facilitated by having real-time access to patient records in the community via laptops.We are looking for practitioners who have a passion for exceptional care, who want to develop their skills and push themselves outside the normal boundaries of practice, and who have a real drive to ensure their patients receive the best treatment possible. Attend patients as both part of the Urgent Community Response crew and alone for follow-up visitsDevelop your skills into a more well-rounded holistic approach to careTake on extended roles such as venepuncture, drug administration, advanced assessment of the acutely ill patient and acute injury assessmentGain experience in generic, orthopaedic, musculoskeletal, respiratory, neurological rehabilitation, and falls preventionGain experience in day-to-day management of the team including taking handovers and helping to make treatment plans during team meetings and at the service user\''s home addressSupervise junior staff and studentsAttend in-service training and have Continued Professional Development opportunitiesBenefits we can offer include a comprehensive induction into the community service, flexible working options, training and development opportunities, and a car lease scheme (available for Band 5 and up).
