The Hospital Assessment and Community Review (HACR) Service is seeking a dedicated and experienced Social Worker to join our Review Team on a part-time basis (3 days per week). About the Service: The Hospital Assessment and Community Review (HACR) Service brings together three specialist teams that work collaboratively to support Havering residents through their journey from hospital discharge to recovery and long-term independence. These teams include Hospital Discharge, the Review Team, and the Discharge to Assess Team. About the Role: As a Social Worker within the Review Team, you will play a vital role in supporting residents following hospital discharge, ensuring they receive the right level of care and support to maximise their independence and well-being. Your responsibilities will include: Completing Care Act Assessments for individuals with ongoing care and support needs following a Home First pathway. Reviewing care and support plans to ensure residents changing needs continue to be met while promoting wellbeing, choice, independence, and safety.
