This service supports people to overcome barriers, build resilience, and achieve their long-term goals. You will work directly with individuals facing homelessness and other complex needs, providing structured, person-centred support.Working Pattern: We work on a 6-week rolling rota of early shift 07:45 till 16:15 and late shifts 12:15 till 20:25, this includes weekend and bank holidays.Key Responsibilities:Deliver person-centred support, empowering individuals to progress on their recovery journey and achieve sustainable independenceBuild strong, therapeutic relationships using a psychologically informed and trauma-sensitive approachFacilitate client meetings and encourage active participation in decision-makingDevelop, implement, and regularly review individual support plans within required timeframesWork one-to-one with clients to achieve agreed goals and positive outcomesEnsure clients are supported to maximise benefits and manage personal finances in line with their plansCollaborate with colleagues to design ..... full job details .....
