Out in the community for assessments and office for case notes and plans

CAR DRIVER REQUIRED

- Person centred care plans, issuing equipment, risk assessments, medication paperwork, Reablement approach, signpost to other agencies, communicate with other teams/ professionals, good relationships with service users/families/ advocates. Knowledge base of Manual handling, NVQ3 equivalent, Trusted assessor trained (Non negotiables) using systems IAS. Have a full driving licence and roadworthy working vehicle.

• Reporting procedure- line manager follow code of conduct, respectful and professional attitude and behaviour
• Any milestones for completion of particular projects
• Whether the services will need to be provided to any other bodies
• Trusted assessor trained to issue and order OT equipment
