You''ll provide assessment and evidence-based treatment interventions to a wide range of older adults across North and West Oxfordshire. We believe in listening and responding as a team, piloting new models, supporting each other and developing and using new care approaches., + Work as an autonomous practitioner in the multi-disciplinary teamundertaking generic Mental Health and specialist Occupational Therapy assessments in collaboration with the service user and their families/carers and presenting back to the wider MDT.+ Work in partnership with service users and their families/carers to ensure the development of a treatment plan to facilitate their safety, promote their well-being and support their independence and inclusion in the community.+ Deliver evidence-based interventions following NICE guidelines to promote recovery and relapse prevention in a variety of settings.+ Manage a complex caseload of patient, with new patient assessments, risk assessment, care planning, reviews and discharges.+ Carry out shifts on the Duty Desk fielding incoming calls, triaging new referrals and assessing new patients.You will be supported by an experienced leadership team from across the professions who facilitate forums for Nursing and Occupational Therapists and hold your development as a priority.
