They will work closely with GPs, practice nurses, HCAs, pharmacists, and other allied health professionals to ensure seamless and holistic care.Responsibilities

Providing evidence-based diabetes management, including reviews, medication titration, and patient educationRunning nurse-led diabetes clinics to empower patients in self-management of their conditionSupporting wider chronic disease management where appropriateActively contributing to the development of clinical protocols and pathwaysParticipating in quality improvement initiatives, audits, and health promotion campaignsSupporting the practice’s role as a teaching and training environment for medical, nursing, and allied health learnersQualifications

Be a qualified Practice Nurse with significant experience in diabetes care and chronic disease managementHold relevant training and qualifications in diabetes management and insulin initiation (or be willing to work towards them)Demonstrate excellent clinical, organisational, and interpersonal skillsBe motivated to contribute ideas, embrace innovation, and support the practice’s vision of continuous improvementHave a patient-centred approach with a strong focus on education, empowerment, and long-term outcomesIn return, the practice offers a welcoming, team-oriented working environment with access to ongoing professional development and training opportunities.
