iPeople SC Solutions is currently recruiting Supported Pathway Officer to work for our client based in Southwark and surrounding areas. The successful post holder will ssist residents in securing their next stage housing, providing resettlement support so that they successfully move into and sustain their new home. The successful post holder will understand the impact of Adverse Child Experiences and the principles of Trauma Informed Care and to provide a holistic key working approach which aims to help residents to recognise preventative action that they can take to break the cycle of homelessness. Duties and Responsibilities: To provide assertive client centred key-working, risk assessment and support planning to a caseload of approximately 10 clients aimed at supporting them to maintain their current accommodation and link into services and meaningful activities which help them to improve their physical and mental wellbeing well-being and life skills Provide a high quality customer focussed service, with clients being treated with courtesy and respect and meet all council standards and targets relating to communication with residents. Assist and motivate residents to reach set goals and progress along an identified clear pathway for housing.
