As a Social Worker, you will work with a diverse range of individuals supporting them to live the life they choose. The work you do will enable carers to continue in their caring roles for as long as they choose and are able to. This role sits within our All Age Continuing Care and S117 Support Team, a specialist service supporting children and adults with complex health and care needs. Working closely with social care, health colleagues and partner organisations, the team helps to secure the right funding, coordinate assessments and make sure people receive the support they need.
