The Role Supporting Hannah in her own home, encouraging and enabling her to fulfil her hobbies and interests whilst adhering to the care plan and implementing the therapy programmes. We require Senior Female Rehabilitation Support Worker(s) to actively encourage Hannah to engage in therapy programmes that will assist her in achieving goals to increase her independence and take on stronger maternal responsibilities, as a mother to her daughter. Whilst also supervising the Care Team, managing the rota, annual leave, and sickness, and working with the Case Manager to ensure that the Care Plan reflects the support needs, and that MAR charts, daily records etc are maintained. Available Hours We are looking for Senior Female Rehabilitation Support Worker for 39.5 hours per week to undertake the following: 1 x 24.25-hour shift, and 1 x 12-hour shift working hands on with Hannah12 hours per month undertaking the Care Coordinator role.
