Overview

Bolton Community mental health teams (CMHTs) provide multi-disciplinary assessment, treatment, and care of individuals with severe and enduring mental health problems. The service is available to residents of Bolton, aged 16 to 70 years, with a diagnosed functional mental health problem. This typically includes service users with:SchizophreniaSevere Affective DisorderComplex Personality DisorderAutism Spectrum ConditionsADHDThere are two CMHTs in Bolton – covering the population of North and South. These are determined by the populations of GP practices and are reviewed in line with the changing population size and profile, and patterns of referrals.Catchment and caseload

North Bolton CMHT

serves the populations of the following GP practices:Halliwell SurgeryBurnside SurgeryMarsden Road Health CentreWaters Meeting Health CentrePike View Health CentreAstley CentreMarket SurgeryKildonan HouseThe catchment population for each Adult Psychiatrist in Bolton, including this post-holder, is 30,000, accounting for GP practice population size and profile, including morbidity, and social deprivation, and referral patterns.The North Bolton CMHT caseload is approximately 800 – 900 patients, shared between two Adult Community Psychiatrists. The post-holder can expect their caseload to be approximately:350 – 400 patients under Care Programme Approach (CPA) with a care co-ordinator50 - 80 patients on non-CPA outpatient only10 – 15 patients who are subject to a Community Treatment OrderThe outpatient clinics (based at Bentley House CMHT in Wigan) are expected to receive a referral rate of 12 new patients per month from the catchment population.It is suggested that the post-holder develop three to four clinics with one new patient and four follow-up patients in each clinic, and eventually develop emergency slots in the future.Working environment and terms
