About the Role As a Health and Wellbeing Practitioner, you''ll manage your own caseload and deliver high-quality psychosocial interventions to adults experiencing substance misuse issues. Working within a multidisciplinary team, you''ll complete assessments, develop SMART care plans and deliver structured one-to-one and group interventions, using both harm reduction and recovery-focused approaches tailored to individual needs. You''ll work closely with healthcare professionals and partner agencies to ensure clients receive holistic support that addresses wider issues including housing, employment, benefits and family relationships. Flexibility is essential within this role.
