You will support individuals with a range of needs, from complex health conditions and communication challenges to those working towards greater independence.Your responsibilities will include: Providing person-centred care and support, both day and night.Assisting with all aspects of daily living, including personal care and household tasks.Supporting individuals to access the community, build independence skills, and stay safe.Working collaboratively with families and professionals to ensure the best outcomes.Using digital tools and apps to promote independence and track progress.
