BACKGROUND
Who We Are
Healthcare Central London Ltd (HCL) is a federation of 31 General Practices and 4 Primary Care Networks (PCNS) in Westminster caring for more than 275,000 registered patients. We provide Community and Primary Care services to our local residents, in addition to a wide range of staffing, management and administrative support to our member practices. By working closely with our PCNs and other key partners, including the Local Authority and community organisations, we help people to stay well and live healthier lives for longer.
We operate Federation-led services including Respiratory and Spirometry hubs; Community Cardiology; Community Dermatology; Community Diabetes service; Access E-hub and Enhanced Access and we support PCNs with managing their NWL single offer contract of services and the National Network DES contract, including ARRS roles.
We are proud to have a unique model of community-led care and support to tackle imbalances and inequality in the system called ‘The Octopus'' and takes us on a journey as part of the Westminster Integrated Neighborhood Team (INT)
We work collaboratively with other partners in the commercial sector including joint ventures; research and consultancy.

ROLE PURPOSE
The post holder will work within the Enhanced Access service on Saturdays, seeing patients face to face on site at one of our hubs.The post holder will demonstrate courageous and critical thinking and is a caring, compassionate and committed experienced nurse who, acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care. They will demonstrate safe, competent clinical decision-making and expert care, including assessment and diagnostic skills, for patients within the general practice and other clinical settingsThe post holder will commit to demonstrating critical thinking in the safe clinical decision-making process. They will communicate and work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required
Key Tasks And Responsibilities
Clinical
Triage urgent care requests if required, consulting with patients or directing them appropriatelyProvide advanced nursing care to patients, including assessment, diagnosis, treatment, and follow-upConduct holistic assessments on patients and ensure appropriate care and action plans based on their needs are clearly recorded and documented.Undertake regular patient reviews.Following assessment where appropriate/necessary and with the patients'' consent refer appropriately to other services and agencies.Perform and interpret diagnostic tests, such as laboratory and radiological testsPrescribe and administer medications in accordance with the law and established protocolsCollaborate with the healthcare team, including GPs, nurses, and allied health professionals, to ensure comprehensive patient careMaintain accurate patient records, including medical histories, treatment plans, and progress notesEducate patients and their families about their health conditions and treatment optionsStay up-to-date on the latest medical research and guidelines relevant to the clinical specialtyMaintain compliance with all relevant clinical standards and regulationsInstigate necessary invasive and non-invasive diagnostic tests or investigations and interpret findings/reportsPromote health and well-being using health promotion, health education, screening, and therapeutic communication skills (i.e. smoking cessation, weight management, alcohol misuse)Refer patients directly to other services/agencies as appropriate

Education and Qualifications
Current Registration of Part 1 of NMC Register
1st Degree/ Master''s level qualification (Advanced Practitioner)
Two year''s post qualification experience - consideration given to ANPs with primary healthcare experience
