Nurse Practitioner Transformation Lead (1.0 FTE, Regular, Inclusive of Clinical Responsibilities)

The Thames Valley Family Health Team (TVFHT) is one of the largest Family Health Teams in Ontario. The Thames Valley Family Health Team is committed to providing comprehensive, collaborative primary health care to over 165,000 patients.The Nurse Practitioner Transformation Lead will play an integral role in advancing TVFHT’s commitment to increasing attachment to teams through Nurse Practitioner as Most Responsible Provider (NP-MRP). This impactful transformation will improve service access through supporting NPs to shift to a formal MRP role and support NPs and teams to achieve full scope. This will involve support of attachment work and developing metrics around complexity, panel distribution, NPMRP models and model outcomes. This role will also support future key service NP initiatives to offer impactful services to improve access and health outcomes of the communities we serve. This role is anticipated to require 0.5FTE allocation, this time allocation will not be consistently distributed, and flexibility will be expected to manage associated accountabilities as well as clinical work. It is expected that this role will guide clinical work and NP MRP transformation at the site level. This position will primarily serve London, Middlesex, Elgin and Oxford communities in alignment with Provincial scope and opportunities.QualificationsCurrent registration as a Nurse Practitioner in good standing with the College of Nurses of Ontario.Master of Nursing (MN) required, Doctor of Nursing Practice (DNP) an assetExperience in primary care as formal MRP required, with a demonstrated record of leadership in clinical practice change or QI.Understanding of quality improvement methodologies, implementation science, or applied health research preferred.Proven ability to lead interdisciplinary teams, mentor peers, and drive evidence-informed practice improvements.Excellent communication skills: both written and verbalLevel of comfort or willingness to become familiar with virtual meeting technologyDuties and ResponsibilitiesDirect NP MRP practice transformation through change management principles and quality improvement frameworksTranslate evidence into practice by integrating clinical guidelines, decision support tools and best practices into day-to-day care deliveryProvide mentorship to NPs in areas of advanced practice, EBP, and clinical decision-makingDevelop and evaluate innovative care models and workflows to improve access, continuity, and interdisciplinary collaborationParticipate in program evaluation in collaboration with leadership and Quality team. Develop practice-level metrics to assess impact of interventions and inform iterative improvementsSupport health equity and population health by embedding equity-informed approaches into clinical transformation strategiesDisseminate outcomes through internal reports, professional presentations, or peer-reviewed publication where appropriate.Participate in provincial, regional and academic partnerships to support knowledge exchange and build capacityMonitor policy developments and emerging trends in healthcare that impact NP scope in primary careContribute to the development of organizational responses to relevant legislative or regulatory changes, supporting the integration of NP-led models of careFoster connections with professional associations (e.g., NPAO, RNAO) and contribute to position statements or submissions that align with the TV FHT’s strategic goals.As needed, represent the FHT and the NP profession in regional or provincial forums, working groups, or stakeholder consultations to advocate for policies that advance access, primary care innovation, and full NP MRP scope of practice.Collaborate with community partners and internal teams to address social determinants of health, improve access for underserved populations, and support both local and system-level gaps in care delivery. This includes identifying priority populations, advocating for equitable resource allocation, and co-developing strategies that bridge service fragmentation across the continuum of care.Occasional travel to partner sites or external stakeholder meetings may be requiredMust have access to reliable transportation as travel between sites is requiredHybrid work may be considered depending on initiative requirements.Salary: $115,105 - $140,372 (dependent on years of experience)To Apply: Interested candidates are invited to submit a detailed resume and cover letter by September 18, 2025 at 4pm.
