As a D2A Care Assistant, you will play a pivotal role in assisting customers with their rehabilitation needs in the comfort of their own homes. Key Responsibilities: Provide safe, reliable, and compassionate support tailored to each customer''s individual rehabilitation programme. Collaborate with the care team, hospital discharge team, and occupational therapist to assess and meet the rehabilitation needs of customers. Respect and promote the dignity of each customer, ensuring care is delivered with informed consent. Monitor and report customer progress towards achieving rehabilitation goals in a professional and timely manner. Ensure clear communication with cluster groups of fellow professionals to aid customers'' rehabilitation. Duties: Work under the guidance of the occupational therapist to design and enhance person-centred rehabilitation plans. Assist customers with personal care, medication management, meal planning and preparation, housework, and more.
