Barts Health NHS TrustThe Pharmacy Department at Barts Health covers a population of 2.5 million. We provide medicines management and medicines optimisation services from four hospital sites across the City of London and East London.We are looking for motivated and dynamic pharmacists to join the department to participate in the rotational band 7 pharmacist programme. The programme includes 9-month rotations across the four sites: Royal London Hospital, St Bartholomew’s Hospital, Whipps Cross Hospital and Newham Hospital.You will work closely with medical and nursing staff and the wider multidisciplinary team on a range of medicines optimisation matters, and to ensure standardisation of medicine policies and procedures where appropriate. You will also be expected to contribute to the research agenda working with staff across the Pharmacy department and student placements.The post holder will work on the wards and deliver quality care to patients at ward level through efficient medicines optimisation. This includes assessment of patients’ own drugs, medicines reconciliation, original pack dispensing at ward level, counseling patients about their medicines, and coordinating the supply of discharge medication. The post holder will work with their ward-based team to continually develop the service while maintaining high standards and achieving best practice to meet patient and Trust needs. Responsibilities
Participate in the rotational band 7 pharmacist programme across four hospital sites.
Collaborate with medical, nursing and multidisciplinary teams on medicines optimisation and policy standardisation.
Contribute to the department’s research agenda and student placements.
Deliver ward-based pharmaceutical care including medicines reconciliation, discharge medication coordination, and patient counselling.
Perform dispensary-based responsibilities as part of the role.
Qualifications / Experience
Motivated and dynamic pharmacists looking to join a large NHS Trust.
Interest in medicines optimisation and clinical pharmacy across multiple sites.
