We are recruiting a highly motivated, innovative and enthusiastic Primary Care Staff Nurse/Pharmacy Tech to join our Health and Justice team at HMP Bronzefield. The service operates on a 7 day week working model to ensure patient care is maintained to a high standard. Our motto is “Caring NOT Judging”.
Key duties include working within Primary Care to provide accessible patient assessment, treatment and referral; monitoring health status; promoting healthy lifestyle changes; implementing discharge plans; coordinating multi-disciplinary care; and collaborating with other staff to develop integrated treatment plans.

Detailed Job Description and Main Responsibilities
Triage patient needs in line with standard operating procedures and national policy (e.g., NICE guidance)
Assess, plan and implement care for patients within care/treatment pathways to promote wellbeing
Facilitate evidence-based practice to ensure effective clinical care
Carry out general nursing duties and maintain contemporaneous records
Support development of integrated healthcare and act as a preceptor/mentor where appropriate
Person specificationEducation and Qualifications
Essential: Registered Nurse Adult
Essential: Numeracy skills for safe administration of medicines
Desirable:
Evidence of relevant Continuous Professional Development
