Clinical Pharmacist role within Central Camden Primary Care Network (PCN) to support service delivery, provide clinical support and guidance.Central Camden PCN comprises 8 member practices working at scale, collaborating with local health and social care partners. The PCN is innovative and delivers services with support from Camden Health Evolution (CHE) Ltd to improve quality of care and outcomes.Main duties

The post holder will work as part of a multidisciplinary team, collaborating with PCN staff and with acute and community providers to ensure safe, accurate, cost-effective, and timely supply of prescribed medication to patients. The role supports the practice workforce, chronic disease management, clinical medication reviews, prescription processes, and medication queries.The post holder will provide leadership and expertise in clinical medicines use, medicines optimisation, and quality improvement (including QOF). Strong communication is essential for liaising with patients, primary care staff, community pharmacists, and other partners.We regret that this position does not provide visa sponsorship. The core responsibilities of the PCN Clinical Pharmacist include, but may not be limited to, the following. Other tasks may be required depending on workload and staffing.Clinical PracticeDeliver pharmaceutical face-to-face consultations, telephone consultations, and home visits to assess and treat patients with long-term conditions.Manage the acute and repeat prescribing reauthorisation process by reviewing repeat requests and ensuring monitoring tests are arranged as needed.Conduct clinical medication reviews and develop medicines care plans in collaboration with patients and carers.Undertake Structured Medication Reviews (SMRs) and shared decision-making for patients on complex polypharmacy, including care home residents.Provide leadership and expertise on medicines and prescribing systems to the multidisciplinary team, patients, and carers.Collaborate as part of a multidisciplinary team to deliver safe and effective pharmaceutical interventions across the PCN.Advise patients on medication optimisation and lifestyle, e.g. inhaler techniques.Support implementation of national prescribing policies and achievement of QOF targets, PCN DES/IIF, QIPP, GP specifications, and other cost-effectiveness initiatives.Conduct medication reconciliation after hospital discharges or other transfers of care, ensuring timely and safe changes.Address medication queries from patients and clinicians per agreed protocols.Collaborate with hospital and community pharmacy colleagues to address discrepancies and supply issues.Develop medicines management plans, including de-prescribing, with clinical colleagues.Use tools such as PINCER to identify at-risk patients and mitigate acute events.Support quality improvement activities, audits, and cost-saving initiatives.Provide expertise to address public health and social care needs, including lifestyle advice and tackling health inequalities.Support antimicrobial stewardship to reduce inappropriate antibiotic prescribing.Collaborate with PCN colleagues and community pharmacy teams to integrate general practice.Attend local meetings to improve access to medicines or repeat prescribing processes as required.Attend and participate in clinical and multidisciplinary meetings as necessary.CommunicationUse sensitive communication to ensure patients are fully informed and consent to treatment.Anticipate and address communication barriers; act as an advocate for patients and colleagues.Provide information in accessible formats and refer as appropriate.Communicate effectively with stakeholders including ICBs and other NHS/private organisations.Collaborate with ICB Medicines Management Team pharmacists on prescribing-related matters.Tailor communication to diverse patient needs and cultural backgrounds.Improve understanding of medication adherence and support commitment to prescribed medicines.Liaise with stakeholders for the collective benefit of patients, including patients, GPs, nurses, care homes, and other health professionals.Quality and SafetyComply with GPhC Code of Conduct and keep registration up to date.Maintain accurate patient records in line with legislation and policies.Provide evidence-based care following NSF and NICE guidelines; promote safe prescribing within formulary and national guidance.Maintain standards of care, manage workload, and undertake required training.Assess care delivery effectiveness through self and peer review, benchmarking, and formal evaluation.Participate in quality governance and policy implementation; respond to MHRA alerts and evolving evidence.Utilise audit cycles to evaluate and improve practice; collaborate with ICB clinical teams on healthcare quality.Assess patient care impact and learn from incidents and complaints using structured frameworks.Recognise own competencies and refer to colleagues as needed.Education and TrainingAttend development courses offered by the ICB MMT; pursue CPD and competencies for responsibilities.Participate in formal education programmes (e.g. CPPE pathway, Primary Care Pharmacy Education Pathway).Support education, training, and mentoring of the pharmacy team and trainees.Provide education to the wider primary care team on therapeutics, medicines optimisation, and quality improvement when appropriate.Engage in shared learning across the PCN and with stakeholders.Key RelationshipsInternal PCN Executive Team, PCN Clinical Directors, PCN member practices, PCN workforce.External community pharmacies, care homes, local providers, ICB, federations, PCNs, GP practices, NHSE, CQC, PPGs.Person specification

EssentialAppreciation of primary care prescribing and rational prescribing strategies.Ability to follow policy and provide medication optimisation per guidance.Excellent communication and interpersonal skills; ability to handle confidential information.Ability to work in a multidisciplinary team and autonomously.Excellent IT skills; strong organisational ability; able to prioritise and provide education and mentorship.Reflective practitioner; open to innovation; able to work under pressure; attention to detail.DesirableAbility to analyse prescribing data; develop and implement policies; motivate and develop colleagues; strong leadership.Experience in primary care and with IT systems (EMIS Web, Docman, Accurx).Other requirementsAbility to travel and work flexibly; attend mandatory training.Disclosure and Barring Service (DBS) check.KnowledgeKnowledge of PCNs and General Practice; evidence-based healthcare principles; pharmacy law and ethics; local and national medicines policies.Understanding of clinical governance in primary care.QualificationsMasters degree in pharmacy (MPharm) or equivalent; GPhC registration; clinical diploma; CPD; independent prescriber or working towards it.Desirable: postgraduate diploma or Royal Pharmaceutical Society membership.ExperienceMinimum 3 years post-registration UK experience; able to work independently or in a team; proven communication with patients and stakeholders.Desirable: experience in primary care; managing acute and long-term conditions; experience with NHS IT systems.AptitudesEnthusiastic, proactive, professional, and calm; able to refer for senior advice when needed.Strong time management, attention to detail, and openness to new ways of working.Capacity to work under pressure and to understand patient needs within the wider team.Additional notesDisclosure and Barring Service check and UK registration requirements apply; annual updates and mandatory training may be required.
