Personal Advisors - Leaving Care Service

Description We seek a dynamic Personal Advisor Looked After Leaving Care Service. This role is significant as it is designed to support care leavers successfully transitioning to adulthood. Your contribution to the assessments, formulation, implementation, and review of Pathway Plans through direct interventions and full engagement of the young person will have a profound impact. Responsibility Provide coaching and motivational support to enable young people to build the confidence and self-esteem to be successful in adulthood. Safeguard and protect care leavers as they transition to independence through regular contact. Qualification Essential GCSE Grade (or equivalent) in English Language and Mathematics. Degree level or extensive experience directly working with children and young people. Advanced Microsoft Office (Word, Excel, Outlook), social media and CRM. Qualifications, Knowledge, and Experience Requirements - Criteria to be Evaluated at the Shortlisting Stage. Successful candidates will have: Experience providing personal advisors for eligible, relevant and former relevant young people, as required by the Leaving Care Act 2000 and volume 3 of the Children Act 2010. Experience in producing and reviewing the pathway plan and regular 2-monthly visits to care leavers. Able to help young people contribute to assessing their needs before their transfer to the Leaving Care Team. Able to review and discuss the pathway plan for 16/17-year-olds with CLAFH colleagues. ..... full job details .....
