Role PurposeThe post holder will work within the Enhanced Access service on Saturdays, seeing patients face to face on site at one of our hubs. The post holder will demonstrate courageous and critical thinking and is a caring, compassionate and committed experienced nurse who, acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care. They will demonstrate safe, competent clinical decision-making and expert care, including assessment and diagnostic skills, for patients within the general practice and other clinical settings. The post holder will commit to demonstrating critical thinking in the safe clinical decision-making process. They will communicate and work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required.
ResponsibilitiesClinical
Triage urgent care requests if required, consulting with patients or directing them appropriately.
Provide advanced nursing care to patients, including assessment, diagnosis, treatment, and follow-up.
Conduct holistic assessments on patients and ensure appropriate care and action plans based on their needs are clearly recorded and documented.
Undertake regular patient reviews.
Following assessment where appropriate/necessary and with the patients’ consent refer appropriately to other services and agencies.
Perform and interpret diagnostic tests, such as laboratory and radiological tests.
Prescribe and administer medications in accordance with the law and established protocols.
Collaborate with the healthcare team, including GPs, nurses, and allied health professionals, to ensure comprehensive patient care.
Maintain accurate patient records, including medical histories, treatment plans, and progress notes.
Educate patients and their families about their health conditions and treatment options.
Stay up-to-date on the latest medical research and guidelines relevant to the clinical specialty.
Maintain compliance with all relevant clinical standards and regulations.
Instigate necessary invasive and non-invasive diagnostic tests or investigations and interpret findings/reports.
Promote health and well-being using health promotion, health education, screening, and therapeutic communication skills (eg smoking cessation, weight management, alcohol misuse).
Refer patients directly to other services/agencies as appropriate.
RequirementsEducation and Qualifications
Current Registration of Part 1 of NMC Register
1st Degree/ Master’s level qualification (Advanced Practitioner)
Two year’s post qualification experience – consideration given to ANPs with primary healthcare experience
