District Nurse Team Lead / Community Nursing Team Lead- SDHandC – Leatherhead PCN. An opportunity to join the Leatherhead PCN team as a Band 7 Community Nursing Team Lead on a full-time (37.5 hours/week), permanent basis. The successful candidate would join an established community nurse team working closely with the wider multidisciplinary team across the PCN and Surrey Downs Health and Care. There is a strong focus on staff support and development, with opportunities for further learning. Lead and manage a community nursing team within the District Nursing Team, delivering high-quality, person-centred care in the community.Oversee assessment, planning, implementation and evaluation of care to agreed standards.Provide staff development and contribute to clinical governance and risk management strategies.Promote integrated care, support workforce planning, change management, resource management and contribute to KPIs and audits.Lead and facilitate educational programmes for basic, post-basic and in-service training in collaboration with the Learning and Development Team.Lead and facilitate patient-focused, innovative, research-based practice in line with Surrey Downs Health and Care objectives.Support safeguarding, collaboration with multidisciplinary teams, and the delivery of high standards of care to patients and staff.Qualifications and experience

Band 7 leadership position with responsibilities for district/community nursing care provision and team management.Experience in leading and facilitating clinical governance, education programmes, and development of staff within a community setting.Additional information
