Project Worker / SEMH Support Practitioner Location: Wyethnshawe Contract: Temporary, on going (Potential of longer extension) Hours: 28 hours per week (Monday to Thursday, 8:30am - 4:00pm) About the Role We are seeking an enthusiastic, compassionate, and dedicated SEMH Support Practitioner to join the Base 12 team. Base 12 is a specialist programme designed to support young people aged 16-25 with an Education, Health and Care Plan (EHCP) for Social, Emotional and Mental Health (SEMH) needs. The programme helps learners who are not yet ready to progress directly onto a vocational college course by developing their confidence, resilience, independence, employability, and social skills. Working closely with teaching staff and the Pastoral and Wellbeing Coordinator, you will provide pastoral, behavioural, and learner engagement support both inside and outside the classroom. You will play a key role in helping learners overcome barriers to education, achieve their personal goals, and successfully progress into further education, training, or employment. Key Responsibilities Provide pastoral, emotional, and behavioural support to learners with SEMH and additional learning needs. Build positive and professional relationships with learners, acting as a trusted mentor and role model. Support learners to develop confidence, resilience, independence, and social skills.
