Job Title: Support Coordinator Location: Southwark, London Contract: 24 Weeks Hours: 36 hours/week About the Role We are currently recruiting for an experienced and motivated Support Coordinator to join a team based at the Southwark Resource Centre, supporting adults with a range of needs including learning disabilities, autism, sensory needs, physical disabilities, dementia and age-related conditions. This is an excellent opportunity for an experienced health and social care or SEND professional who is passionate about person-centred support, promoting independence and improving outcomes for the people they support. Working closely with the Centre Manager and wider professional network, you will coordinate and lead a team delivering individualised, outcome-focused support across the resource centre, sister locations and community settings. Key Responsibilities Lead, coordinate and support a team delivering high-quality, person-centred services. Promote independence, wellbeing, resilience and positive risk-taking. Develop and coordinate meaningful activities and opportunities for individuals to learn new skills and build relationships. Ensure support plans and Positive Behaviour Support (PBS) plans are implemented, reviewed and regularly updated. Provide effective supervision, line management, coaching and performance management to staff. Promote and maintain high standards of safeguarding, health and safety and risk management.
