Job Responsibilities

  • Participate as part of a team managing a new and dynamic service supporting individuals with complex needs, including mental health, physical health, and substance misuse issues.
  • Assess and manage risks for clients and the service, ensuring safety and well-being at all times.
  • Deliver high-quality care, support, and housing management in accordance with organisational policies and procedures.
  • Work collaboratively with partner agencies to provide holistic, wrap-around support tailored to individual needs.
  • Develop and implement client-centred support and risk management plans, aiming for suitable move-on options within six weeks.
  • Engage with clients to build positive relationships, motivate progress, and encourage participation in their recovery journey.
  • Manage challenging situations effectively, ensuring safety for clients and staff, and report safeguarding concerns promptly.
  • Maintain accurate and timely case records, update IT systems, and produce reports as required.
  • Contribute to a positive, inclusive working environment that values diversity and promotes equality.
  • Participate in rota shifts, including evenings, weekends, and bank holidays, to ensure continuous support coverage.
  • Support clients with housing benefit claims and other administrative tasks related to tenancy management.



Required Skills & Qualifications

  • Understanding of the needs of homeless people and experience working in a face-to-face support environment.
  • Experience working with individuals with complex needs, including mental health, physical health, and substance misuse issues.
  • Knowledge of trauma-informed care and move-on options for clients.
  • Excellent communication skills, with the ability to connect, motivate, and adapt approach to diverse clients and stakeholders.
  • Strong decision-making skills, with the ability to assess risks and respond effectively in challenging situations.
  • IT literacy, including proficiency with Microsoft Office packages and database management.
  • Ability to work independently and as part of a team, demonstrating self-motivation and a proactive attitude.
  • Relevant qualifications or certifications in health, social care, or related fields are desirable.
  • Commitment to safeguarding, equality, and diversity principles.
