As Team Leader - Step Service, you''ll lead a team delivering high-quality accommodation-based support to people with complex health and support needs, helping them move towards safe, sustainable independence. You''ll oversee day-to-day service delivery, referrals, risk and safeguarding, while coordinating personalised support and move-on plans. Working closely with Connection Support, hospitals, health teams and other agencies, you''ll build strong partnerships and ensure residents receive the right support at the right time. Main Purpose Of Job: The Step service provides two differing services: Step Up service is a model providing accommodation based support and assessment to prevent people having the need to go into hospital. Step Down provides accommodation based support and assessment for people who are being discharged from the hospital using the discharge to assess model. This provision in Oxfordshire is run in partnership between Homeless Oxfordshire and Connection Support.
