This candidate should be a compassionate Social Worker with excellent customer service skills to assist our clinic patients in the area.This role involves in-person visits, traveling to patients'' homes and facilities daily.
This is a salary-based position, with the opportunity to receive a monthly bonus. The role involves working with patients to complete advance care planning, cognitive assessments, placement, and providing general resources. Responsibilities include:Initiating Advance Care Planning for patients across assigned Care Teams, including facilities.Completing Daily Comprehensive Assessments.Developing Daily Care Plans.Documenting Chronic Care Management activities daily.Meeting Daily Productivity Goals.Coordinating discharges at contracted facilities.Arranging patient placements.Continuing coverage for acute visits, medication changes, etc.Building and maintaining professional relationships with referral sources.Maintaining strong communication with referral sources, patients, families, and Care Teams.
