Eden Brown Synergy are currently looking for an experienced Qualified Social Worker to join the Mental Health Team at Bromley Council.Duties and ResponsibilitiesManage an allocated caseload of adults experiencing mental health needs.Undertake comprehensive assessments, reviews, and care and support planning in line with the Care Act 2014.Assess risk and identify appropriate interventions and support to promote independence and wellbeing.Undertake Mental Capacity Assessments and Best Interests decision-making where required.Respond appropriately to safeguarding concerns and work in accordance with statutory requirements.Work collaboratively with service users, carers, health professionals, housing, and other partner agencies.Develop person-centred, outcome-focused support plans based on individual needs and strengths.Maintain accurate case records, assessments, reviews, and reports.Work effectively within a multi-disciplinary environment to achieve positive outcomes for service users.Working PatternHybridPay Rate 32 p/h UmbrellaEssential RequirementsSocial Work England (SWE) registration.Social Work qualification.Previous experience relevant to Adult Social Care / Mental Health Social Work.Knowledge of the Care Act 2014, Mental Capacity Act 2005, and safeguarding procedures.Experience undertaking assessments, reviews, risk assessments, and care planning.Strong communication and multi-agency working skills.BenefitsSupport from an experienced recruitment consultant.Access to ..... full job details .....
