The Hospital Social Work Team fulfils the critical link between our local hospitals and community services to help people get home as soon as they are able. The service identifies strengths and resources to ensure people get the help they need, working with people and their families to achieve long-term outcomes.This role offers an opportunity to build on the success of our existing teams and create a model that delivers long-term outcomes for the people of Bridgend.
