Conduct comprehensive assessments and provide psychosocial interventions within a structured 12-week framework.Engage in duty work and collaborate with colleagues on discharge coordination to efficiently manage caseloads.Work as part of a multi-disciplinary team, including clinicians, social workers, and support workers, to deliver patient-centred care.Communicate effectively with patients and their families to provide support and education about their care and treatment plans.Maintain accurate and timely records, documenting patient progress and treatment outcomes.Qualifications
