We are looking for an Adult''s Social Worker to join a Continuing Healthcare Team. This role requires a Social Work Qualification with a minimum of 2 years post qualified experience. About the teamAs a Social Worker in this team, completing assessments and reviews within the CHC framework is a key aspect of your day to day responsibilities. You will be required to attend dispute resolution meeting and present cases to panel when needed. The team is given regular supervision from management to ensure support and collaboration on complex cases.
