Eden Brown Synergy are currently looking for experienced Qualified Social Workers to join the Adult Safeguarding & Gateway Front Door Team within the London Borough of Lewisham.

Duties and Responsibilities

• Respond to and assess referrals received through the Adult Safeguarding & Gateway Front Door.
• Assess the social care and safeguarding needs of adults and identify appropriate interventions.
• Undertake safeguarding enquiries and risk assessments, working to achieve person-centred safeguarding outcomes.
• Work closely with colleagues, partner agencies and other professionals to gather information and manage risk effectively.
• Identify appropriate pathways and support for adults requiring social care intervention.
• Apply knowledge of the Care Act, Mental Capacity Act and relevant adult safeguarding legislation.
• Participate in a fast-paced front-door service, responding to referrals and professional enquiries within required timescales.
• Maintain accurate and timely case records and ensure appropriate information is recorded.
• Work collaboratively across Adult Social Care and with relevant external agencies.
• Working Pattern: Minimum 2 days per week office based.

Pay Rate

• -32 p/h Umbrella

Essential Requirements:

• Social Work England (SWE) registration.
• Social Worker qualification.
• Previous experience working within Adult Social Care.
• Experience of Adult Safeguarding and/or a Gateway Front Door service.
• Strong assessment, risk management and written communication skills.
• Good knowledge of relevant adult social care and safeguarding legislation.
