JOB PURPOSE

To provide team leadership in co-ordinating the day to day provision of integrated wellbeing, mental health and substance misuse healthcare to service users in a custodial setting and maintain the overall standards which will include a single point of referral assessment and delivery of care within the framework of The Care programme Approach and recovery agenda. This will involve both clinical responsibilities of managing a small caseload as well as day to day operational responsibilities and will require the post holder to deputise for the clinical lead as and when appropriate/required.

KEY DUTIES AND RESPONSIBILITIES:

• To support the clinical lead in the implementation of integrated care pathways including appropriate service user assessments and models of care within a stepped care mental health model.
• Co-ordinate care delivery, across the mental healthcare pathway e.g. referrals management, allocations of named worker/care co-ordinator, ensuring seamless continuity of care across the stepped care model dependent on service user need.
• To demonstrate the application of appropriate procedures and standards.
• To assist in the general management of the clinical area, with responsibilities for managing attendance.
• Lead on caseload and management supervision to monitor named nurse / care co-ordinator standards of service user care and documentation.
