Overview

Your role will be so important in supporting people who are entering the last phase of their lives to talk about what is important to them and helping them to navigate how their wishes and goals can be achieved. You will also support families and friends as well as providing expert care that will support a dignified death. You will offer the people we support your skills in symptom control, social, emotional and spiritual care, in addition to providing advice, support and education to the primary health care team and hospital staff where appropriate, as part of an interdisciplinary team.Responsibilities
