We are currently working with a client in Liverpool who is looking to recruit an experienced Social Care Assessor to join their Adult Access Team.As a Social Care Assessor, you will be responsible for assessing individuals and, where necessary, providing short-term support or reablement to promote independence. Where longer-term intervention is required, you will complete robust assessments to support a smooth transition into Locality Teams for ongoing support.You will work with adults aged 18+ and manage a varied caseload with the support of supervision and guidance, ensuring all work is carried out in line with relevant legislation, policies, and procedures.Key Responsibilities: Manage and prioritise a varied caseload effectivelyComplete Reablement, Needs, Risk, Mental Capacity, and Best Interest Assessments, as well as reviewsCommission appropriate services including domiciliary care, day placements, carers support, assistive technology, and minor adaptationsWork collaboratively within a Multi-Disciplinary TeamAttend daily huddles and participate in duty responsibilitiesRepresent the Adult Access Team at external meetings when requiredApply the Care Act''s Prevent, Reduce, Delay principles to promote independence and wellbeingKey Requirements: Previous experience working within Adult Social CareExperience completing strength-based assessmentsKnowledge of relevant legislation including the Care ActAbility to manage a varied caseload effectivelyExperience working within a ..... full job details .....
