The successful candidates will be working with individuals in the community who are experiencing acute mental health crises, helping to prevent hospital admissions by providing treatment and support within the home. You will play a vital role in assessing, supporting, and managing service users with mental health needs in crisis, assisting them to remain in their community and avoid hospitalisation. Your work will involve a combination of assessments, care management, medication oversight, and liaising with various stakeholders, including families, other mental health services, GPs, and carers.
