Social Worker – Children with Disabilities Team - Herefordshire CouncilLocation: Hereford
Pay/Salary: £41,363 - £45,777 + £10,000 Welcome Payment
Office Working: 2-3 Days per week
Contract: PermanentWe are currently working with Herefordshire Council to find an experienced Social Worker for their Children with Disabilities Team within Children’s Services. With a stable management team and a mostly permanent workforce, Herefordshire are looking for someone with solid experience to be able to pick up a caseload and support their services.

Duties

• Undertake statutory visits and completing S17 Assessments and S47s, parenting and sibling assessments, evaluating needs of the child and family.
• Developing care plans for children and families, including provisions such as respite and short breaks.
• Working closely with partner agencies, such as Health, Education, and Police.
• Chair CIN, Core Group and professionals’ meetings, as part of the multi-disciplinary team.
