Hospital Discharge Support Worker (Mental Health)About the Role

We are seeking a committed and experienced Hospital Discharge Social Worker to join our team on a short-term contract. The post holder will play a vital role in supporting patient flow from psychiatric units, helping to relieve increasing hospital discharge pressures and ensuring timely and safe transitions from hospital to the community.

This role also includes supporting step-down arrangements and assisting localities with complex care discharges for patients aged 65 and over, ensuring that individuals receive the right care, in the right place, at the right time.

• Facilitate safe and timely discharges from psychiatric units.

• Support hospital discharge step-down arrangements, ensuring continuity of care.

• Work closely with Locality teams to manage complex care discharges for over-65 patients.

• Liaise with hospital staff, community services, social care, and families to ensure appropriate follow-up support.

• Contribute to reducing delayed transfers of care and improving patient outcomes.

We are looking for someone with:

• Experience in hospital discharge planning, mental health, or adult social care.

• Strong understanding of psychiatric services and/or older people’s complex care needs.

• Excellent communication and partnership-working skills across multi-disciplinary teams.

• A proactive and flexible approach to problem-solving in high-pressure environments.

• Commitment to promoting independence, dignity, and person-centred care.
