The service supports early discharge from hospital and ongoing rehabilitation at home, working across ESD, stroke and neuro pathways. Role Summary The postholder will provide a specialist Speech and Language Therapy service to adults with communication and swallowing difficulties arising from neurological conditions. You will work as part of a multidisciplinary team delivering high-quality, patient-centred care across community pathways. Key Responsibilities Deliver specialist SLT assessment and intervention for communication and dysphagia post-stroke Manage and prioritise a complex community caseload independently Work collaboratively across ESD, stroke, and neuro MDT pathways Provide advice, education and support to patients, carers, and wider teams Assess and recommend AAC (alternative and augmentative communication) aids Support and supervise junior staff and contribute to team development Deliver training and contribute to service improvement and CPD initiatives Maintain accurate clinical records ..... full job details .....
