Registered Mental Health Nurse

Overview
Work as an effective professional in a multidisciplinary Older Adults Community Mental Health Team. To prioritise and respond to urgent referrals for assessments as required and deliver high quality health and social care within the Care Programme Approach framework and to participate in the duty rota. Hold care coordinator responsibility for a complex caseload of services users and their carers. Responsibility for assessing individual care needs and the planning, implementation and evaluation of care plans, in line with Oxleas Foundation Trust CPA policy. Provide supervision, mentorship and leadership for support workers and Student Nurses, as appropriate. Demonstrate understanding and use of clinical supervision to reflect upon and modify own and others practice.Key responsibilities
Coordinate and manage a complex caseload with particular focus on maintaining high standards of professional and clinical practice in all disciplines.Ensure that all organisational policies, procedures and guidelines are adhered to and contribute to the development of local ones.Ensure the development of services that are comprehensive, flexible and promote a culture of service users and carers'' involvement at every stage of the care pathway.Have experience of working in a complex and challenging environment with a particular focus on:Ensuring a culture of effective risk management in the team.Taking a lead on management of complex cases and supporting less experienced staff with the management of such cases.Responding to serious untoward incidents as appropriate and supporting any subsequent investigation.Responding to Safeguarding Adult alerts as appropriate.Ensuring complaints are investigated fully within time limits, with a proactive approach in line with trust policies.Responding to communications regarding team issues.Promoting and displaying support for the Trust''s policies on Equality and Diversity and the recovery-orientated approach for people with longer term problems.Additional information
