Herts and West Essex Integrated Care Board (HWE ICB) is looking for an experienced Nurse who has specialised knowledge and experience of working with individuals with complex care needs. You will have worked in either a specialised or community setting with experience of the Continuing Healthcare process.Main duties of the job

The post holder will be responsible for validating applications received from a variety of practitioners recommending people for Continuing Healthcare and NHS Funded Nursing Care and act as a link person for continuing care service users and carers.The post holder will be the case manager for people who are within the All Age Continuing Care [AACC] caseload and will provide professional advice to patients, carers and multi-agency colleagues.The role will require a high level of communication and interpersonal skills and the ability to work supportively and effectively with individuals and their families ensuring that they are fully involved throughout the process.If you have the energy, enthusiasm, values and drive that are required for this post we invite you to apply.About us

Qualifications

Registered Nurse (Adult or Mental Health) - with current registrationEducated to degree level in relevant subject or equivalent level qualification or significant experience of working at a similar level in specialist areaPost-graduate degree in Management Studies or equivalentExperience and Knowledge

Experience of working in the NHS/public bodyEvidence of working knowledge of CHCExcellent understanding of the current issues impacting on the NHS, governance, policy and an experience of its implications so as to interpret and communicate them locallySignificant experience of successfully operating in a politically sensitive environmentDemonstrated experience of coordinating projects in complex and challenging environmentsExperience of managing risks and reportingCommunication Skills
