Community Mental Health Meaningful Intervention PractitionerWe are seeking an innovative and independent practitioner to join a developing and nurturing team. You will work within multi-disciplinary recovery-based mental health services and be a core member of the team, providing high-quality meaningful interventions. The role involves close collaboration with community partners, including GPs, staff, and voluntary sector organizations.
The team’s vision is to shift focus from clinical settings to the community, addressing Newham''s population health needs and dissolving boundaries between health and community services.
Responsibilities include providing effective mental health support and care to Newham residents, responding promptly to the needs of adults with mental health issues and their carers within statutory legislation, and considering individual circumstances. You will explore needs using a person-centred approach, assess and manage risks, and plan and deliver outcome-focused interventions to promote independence and social inclusion. You will act as an allocated worker, responsible for community services for mental health service users and carers.
