You will be closely working in our friendly, supportive 0-19 team in West London. This role is designed to improve access to health promoting information for families and streamline referrals into the HV service.We work with families in one of the most diverse boroughs in London to give children the best possible start in life by providing the most up-to-date evidence based support.As part of our skill mix team, you will contribute to the delivery of the national 0-5 years Healthy Child programme across Hounslow, working closely with partners, such as such as GPs, children’s centres and social care, providing seamless care to the population of Hounslow.
