Digital and Transformation Lead (DTL)

tosupport the coordination and delivery of high-quality, innovative primary careservices. The DTL will play a central role in operational leadership, digitaltransformation, and service redesign across the PCN, in line with the NetworkContract DES, ARRS framework, and NHS priorities.Main duties of the job

The post holder will lead on key digital programmes across the PCN, including the implementation and optimisation of Abtrace and EMIS systems. They will act as the PCNs digital and transformation lead at local and ICB forums, helping to ensure digital maturity across all practices.They will also provide operational leadership across thePrimary Care Network (PCN), ensuring services are delivered in line with thePCN DES, Investment and Impact Fund (IIF) and evolving NHS priorities. Thisincludes ensuring systems are in place for clinical governance, qualityassurance and risk management.Supporting the recruitment and induction of ARRS staff, as well as provide line managementsupport to team members such as care coordinators.Additionally they will provide management and administrativeleadership for the PCNs Oversight Group, ensuring effective coordination ofmeetings, follow-up actions, and programme governance. They will be responsible for supporting the development and deliveryof the PCNs priorities, ensuring key decisions and progress are documented andmonitored. This includes maintaining oversight of project documentation,reporting requirements and communication with member practices and externalstakeholders.About us

consists of 10 GP practices basedover 8 sites, caring for a population of over 75,000 patients.Wework together with a range of local providers, including acute trusts,community services, social care, voluntary sector and patient participationgroups, to offer more personalised, co-ordinated health and social care to ourlocal population.Job responsibilities

The post-holder will:Leadthe delivery and continuous development of PCN services in line with the NetworkContract DES, Investment and Impact Fund (IIF), PCN neighbourhood developmentand evolving NHS priorities.Ensuretimely and accurate submissions for DES, IIF, Enhanced Services and otherincome-generating initiatives working closely with clinical and administrativeteams.Designand implement new models of care that maximise the impact of themultidisciplinary team and embed digital tools and pathways effectively.Developand maintain comprehensive Standard Operating Procedures (SOPs), governanceframework and business continuity plans to support business resilience.Leadthe strategic implementation and optimisation ofdigital transformation programmes across member practices, including but notlimited to:- Cloud-basedtelephony- Digitaltriage platforms- EMISHub and workflow standardisationRepresentthe PCN at digital transformation forums and other external networks.Actas the primary liaison with key internal and external stakeholders, includingpractice managers, clinical leads, ICB digital transformation teams, IT supportproviders, and external contractors.Monitorand evaluate the impact of digital initiatives, using data dashboards,feedback, and quality improvement methodologies to inform continuousimprovement.Committo ongoing professional development, including training in digital health,leadership, service redesign and change management.Lead the transformation and operational delivery ofthe Enhanced Access Hub, including digital triage, capacity planning, reportingand continuous service improvement.Supportthe design and implementation of Neighbourhood Models of Care, aligned tointegrated neighbourhood teams and local place-based priorities.Facilitatecross-practice collaboration and standardisation of pathways across the PCN forlong-term condition management, urgent care access and preventative health.Workwith local partners (e.g community services, mental health providers, localauthority teams) to support integrated service models.Drivepatient-facing digital innovations (e.g., online consultation tools, NHS Appoptimisation, patient education platforms) to enhance access andself-management.Championdigital inclusion and ensure equitable access to digital services for diverseand underserved populations.Contributeto the creation and delivery of a PCN-wide digital and transformation roadmapaligned with local Integrated Care System (ICS) strategy.Person Specification

Knowledge and Skills

Significant and demonstrable knowledge and understanding of PCN networks and their prioritiesExceptional organisation and administrative skillsKnowledge of Microsoft Office Software / Outlook / PowerPoint and ExcelGood oral and written communication skillsEMIS Clinical systems experienceA general understanding of the NHS and the way it works both locally and nationally.Values and Behaviour

Degree or equivalent level education OR ability to demonstrate competency at this levelProject Management Certificate (Quality Service Improvement and Redesign (QSIR) or Prince 2)Disclosure and Barring Service Check
